The forms provided below are for your convenience in executing the following transactions:

Investment Change Form - This form is used to make recommendations for fund holders’ investment allocations. Typically, fund holders do not change their investments often; however when appropriate EBCF will accommodate these changes on a quarterly basis.

Grant Recommendation Form - This form is used to make grant recommendations via mail or Fax. Grant recommendations can also be made on-line via the Donor Express portal on this website.

Donor Delivery Instructions - This form contains EBCF banking instructions for sending gifts that are in the form of cash, stocks and bonds. Please contact the Development Department at 510.208.0819 for the acceptance of other types of gifts.
